Pastors Award Certificate

Instructions for use:

Select the award year from the list below to view the certificate.  Save the file to your computer, then open the saved file and complete the required information: Church NAME and ID NUMBER, YOUR NAME, and the DESIGNEE (student’s name.)  After you complete the information, tab out of the last box, save again, and it should retain the information you entered.

Email the completed form to churchrelations@ebasd.com, or print it, complete it, and mail it to:

Please make note of these deadlines

* To redeem certificate(s) for an academic term beginning in August, certificate(s) must be submitted by May 15.
* To redeem certificate(s) for an academic term beginning in January, certificate(s) must be submitted by October 15.
* Up to five (5) certificates can be combined and submitted together for an academic term.
